Our Mission

At HOME Street Medicine, we are bridging the gap in health care for unsheltered individuals in Leon County. Founded by passionate Physician Assistant Practice students and faculty in 2022, we provide health screenings, build relationships, and connect vulnerable populations to primary care — all while learning together and empowering each other. 

Through partnerships with local agencies, we meet people where they are, addressing not just medical needs but the systemic barriers they face. Our work isn’t just about providing medical attention— it’s about fostering compassion and understanding among future health care providers, ensuring they’re equipped to serve the underserved with dignity and respect.
 

HOME Street Medicine's Goals

     1. Expand Access to Care

  • Increase outreach efforts to more unsheltered encampments and underserved areas in Leon County
  • Provide on-the-spot medical attention, patient education, and referrals to ensure comprehensive support for individuals experiencing homelessness

     2. Empower Student Learning

  • Offer meaningful opportunities for students to apply clinical knowledge in real world settings, fostering confidence and cultural humility
  • Promote interprofessional collaboration between PA, MD, and pre-health students to build a team-based approach to care

     3. Enhance Community Impact

  • Distribute essential items and meals to meet the immediate needs of individuals and families
  • Establish sustainable partnerships with organizations like the Kearney Center and Grace Mission to broaden the program’s reach

     4. Advocate for Systemic Change

  • Raise awareness about homelessness and healthcare inequalities among students and the broader community
  • Equip participants with the skills and motivation to champion healthcare reform throughout their careers

     5. Ensure Program Sustainability

  • Secure funding through grants and donations to sustain and expand services
  • Develop a long-term strategy to scale operations, including additional team members and resources

Get Involved with HOME Street Medicine!

Volunteer With Us!
We are currently looking for undergraduate, medical, and PA students who are interested in volunteering with our upcoming HomeStreet Medicine outreach rounds. During outreach rounds, volunteers will assist with blood pressure checks, blood glucose checks, distributing essential supplies, and engaging with patients under faculty supervision. We have current volunteer opportunities in local encampments and with Grace Mission.

To volunteer, students must:

  1. Have a Home Street Medicine shirt (shirts are available for purchase, and we can provide them prior to volunteering). 
  2. Complete the Home Street Medicine training module on Canvas and pass the post-test with a score of 70% or higher.

Support Our Mission by Donating

Community contributions help Home Street Medicine provide essential medical attention, supplies and outreach services to individuals experiencing homelessness. With the help of generous donations from individuals, local businesses and health care partners, HOME provides medical supplies, hygiene kits and necessities for those in need. These contributions help volunteer providers and students offer free medical attention to underserved populations.
