How to Report Discrimination, Bias, or Related Incidents to the Council on Diversity and Inclusion

The Council on Diversity and Inclusion (CODI) offers a reporting service available to all members of the College of Medicine community, including students in all programs, residents and fellows, as well as faculty and staff at the Main Campus and all Regional Campuses. Please note this form is for individuals who have an affiliation with the College of Medicine. Note that the CODI Subcommittee is required to report incidents of sexual harassment or sexual assault to the Office of Equal Opportunity and Compliance under Title IX and FSU Policy 2-2.

Reporter Confidentiality

Reports are submitted through Qualtrics and do not collect IP addresses. In general, all reports will be treated as confidential and shared only on a “need to know” basis as necessary to achieve a prompt and appropriate institutional response. Reporters are free to provide contact information or not; however, it may be more difficult to follow up or satisfactorily address the concern if the reporter chooses not to provide contact information.

Reasons to Report

All members of the College of Medicine community are encouraged to report incidents of discrimination, bias, and related concerns, even if they prefer that no response is taken. Simply reporting concerns will allow for a more accurate understanding of the climate of the College, and in turn will allow for the creation of targeted interventions to address needed areas of improvement.

Medical Student Mistreatment

To ensure an appropriate institutional response, reports that involve mistreatment of a medical student will be shared with the Deans of Student Affairs and Admissions unless the reporter elects to not have Student Affairs involved, or if a member of Student Affairs is involved in the complaint. In such an instance, the Subcommittee will discuss the complaint with another senior-ranking member of the executive administration, such as the Senior Associate Dean for Academic Affairs or the Dean of the College of Medicine.

Report Received

Once a report is received, the Subcommittee will review the report to develop a fair and impartial understanding of the reported incident(s).

Reporter Updated

If further exploration is warranted, the reporter (if known) will be alerted that a review is underway.

Assessment

The Subcommittee will conduct a review of the incident. As part of this process, the Subcommittee may elect to privately discuss incidents of concern with individuals named in a report. However, if the reporter provides contact information, then their preferences regarding who may or may not be consulted, along with potential responses to the report should, except in the most exceptional cases, be determinative.

Recommendations

If a determination is made that mistreatment has occurred, the Subcommittee may make a formal written recommendation to the appropriate executive unit, if it deems this appropriate for the case at hand.

Reporter Updated

Upon completion of the review, the Subcommittee will update the reporter as to the status, as appropriate.

Database Updated

In an effort to monitor and track trends in possible incidents, de-identified, composite reports will be generated to identify themes as well as trends over time. These de-identified summaries will be shared on a regular basis with leadership of the College, including the Council on Diversity and Inclusion, the Office of the Dean and the Senior Associate Dean for Academic Affairs, along with other relevant leadership. The purpose of sharing de-identified summaries is to improve the climate and ultimately advance the mission of the College.

Subcommittee to Address Reports of Discrimination or Bias

Submitted reports are reviewed only by the Subcommittee to Address Reports of Discrimination or Bias, not the entire Council. Members of the Subcommittee are selected by the Council, serving one-year renewable terms. Our Subcommittee currently consists of Mytrice Allen, Susan LaJoie, Cheryl Porter, George Rust and Susan Salahshor, who serves as the Subcommittee Chair.
