IMS Experiential Seminar Placement Form

The Interdisciplinary Medical Sciences (IMS) B.S. Degree program offers three majors, 1) Community Patient Care, 2) Health Management, Policy, and Information, and 3) Clinical Professions which require health related experiential learning hours as part of the degree requirement. Undergraduate transfer students seeking admission or current FSU students wishing to change their major to one of the three majors in the IMS B.S. Degree Program must complete the IMS Experiential Seminar Placement Form.  Failure to submit by the deadline will affect the approval decision. Only one IMS Experiential Seminar Placement Form needs to be submitted per term.

Note: All students wishing to transfer, change major, seek a Second bachelor's degree or readmission into any of the IMS majors MUST submit the IMS Experiential Seminar Placement Form. 

Transfer, Second Bachelor's Degree, or Readmission Students

Students seeking admission or readmission into FSU must first apply to the University through the FSU Undergraduate Office of Admissions Application process. This is a separate application from the IMS Experiential Seminar Placement Form.

  • The FSU Application will create an identification number, the FSU EMPLID, which you will need to complete the IMS Experiential Seminar Placement Form.
  • Be sure to use the same email address for both FSU admission application and IMS Experiential Seminar Placement Form.
  • Deadlines to submit IMS Experiential Seminar Placement Form:
    • Spring Admission: November 1
    • Summer Admission: March 1
    • Fall Admission: June 1
  • Please note that completion of this application does not guarantee admission into FSU or the IMS Degree program.
  • You will be contacted by the IMS B.S. Degree Program Mapping Coordinator as soon as your application is reviewed.


Major Change for Current FSU Students 

Students interested in changing their current major must submit the IMS Experiential Seminar Placement Form before the major change can be approved.

Note: Any student who misses the deadline will have to wait until after grades post to be considered for the major change.

  • Please note that completion of this form does not guarantee acceptance into the IMS B.S. Degree Program.
  • Any mapping holds placed by your current major will not be removed unless your major change has been approved by the IMS B.S. Degree Program Mapping Coordinator.
  • You will be contacted by the IMS B.S. Degree Program Mapping Coordinator as soon as your application is reviewed.
