Work Study Employment

We are pleased that you will be joining the College of Medicine. In order for us to get your paperwork completed in a timely manner, you will need to complete the following forms. 

1. Go to the following FSU web site and complete the "wizard".  As you complete the "wizard", these forms will be created to print out and sign.  https://hrapps.fsu.edu/formswizard2/
   
   You will also need a copy of:
2. Your Federal Work Study Employment Authorization Form (this will need to be completed by your supervisor
3. Your driver’s license
4. Your social security card
   
   For Foreign National Students ONLY, you also need the following:
5. Foreign National Form [pdf] - download, completed, sign and print.
6. Passport, issued by candidates home country
7. U.S. Visa
8. Social Security card or receipt of application
9. Visa documents

Once you have everything in order, please take the originals to the Federal Work Study Office and turn in a copy of these forms to your supervisor.
