College of Medicine Employment
A&P Employment

We are pleased that you will be joining the College of Medicine. In order for us to get your paperwork completed in a timely manner, you will need to complete the following forms.

  1. Go to the following FSU web site and complete the "wizard".  As you complete the "wizard", these forms will be created to print out and sign.  https://hrapps.fsu.edu/formswizard2/
  2. Receipt of Distribution of Property [pdf]
  3. Complete the 2007-2008 confirmation statement [pdf] indicating whether you will or will not be involved in any outside employment for the 2007-2008 Academic year. If you indicate that you will be involved, please also complete the Outside Employment Staff [pdf].
  4. Background Check Request [pdf] - If required, only complete the Person Information Section. (Your employment email that you received from HR will indicate if this is required.)
  5. A copy of your driver’s license
  6. A copy of your social security card
  7. A copy of your Degree certification showing the latest degree awarded (Official transcripts are also accepted)
  8. Please sign the offer letter that was originally attached to the employment email that you received from HR.
  9. Please sign the contract that was originally attached to the employment email that you received from HR.

Once you have everything in order, please either mail or fax these forms to Human Resources as soon possible so that we can ensure that you will be in our system upon arrival. If you have any questions, please do not hesitate to contact our Human Resources department at 850-644-8155.
