1. _____ * 08/09 FAFSA completed. (If you want to be considered for COM Scholarships, you MUST place your parents ’07 tax information on your FAFSA. If you are 28 years old or older or if you are married, we only need your tax information and/or your spouses’ tax information on your FAFSA) (School ID #001489)
    

2. _____ * Summer Application submitted online (Check MED)
    

3. _____ * All requested documents, from your Financial Aid Student Toolkit, submitted to the College of Medicine Financial Aid Coordinator
    

4. _____ * Complete and submit the College of Medicine Foundation Scholarship Application before June 15, 2008
    

5. _____ * If you are awarded with any OUTSIDE scholarships, please forward a copy of your award letter to the College of Medicine Financial Aid Coordinator

Please be advised that if #1-5 are not completed, there WILL be a
2 week DELAY in receiving your Financial Aid for the summer.
