Transition Care Clinic Now Open
Geriatric services are now being provided by John Agens, MD, and Niharika Suchak, MD, Associate Professors of Geriatrics, and Ken Brummel-Smith, MD, Charlotte Edwards Maguire Professor and Chair of Geriatrics through Tallahassee Memorial Healthcare’s Transition Center. A partnership between Capital Health Plan and Tallahassee Memorial Healthcare, the Transition Center, targets those being discharged from Tallahassee Memorial Hospital who don’t have a primary physician, don’t have insurance, or can’t see their own doctor in a reasonable period of time after discharge from the hospital. A care plan is developed with the patient after a thorough evaluation with attention to medication management, physical function, cognitive function, and social functioning. Care is then transferred to a primary care doctor in our community in about eight weeks. The goal is to make the patient a partner in their own care and to reduce the need to go back to the hospital.
Stephen Quintero, MD, Assistant Professor, Family Medicine and Rural Health, is the Transition Center’s Medical Director. He oversees all care at the Transition Center, including non-geriatric care. The Transition Center is located at 3333 Capital Oaks Drive next to FSU COM’s Tallahassee Regional Campus. Appointments can be made by calling the Transition Center at 850-431-4470. Hours are Monday through Friday 8AM to 5PM.
